About Us

Brighton and Hove is a vibrant and exciting place to live and work and an opportunity has arisen to join the Hospital Responsive Service social work team.

Both the acute and community arms of our wider team have seen their roles significantly evolve in recent years as we have been at the forefront of changes with the hospital’s Discharge to Assess model. This has meant our team work with people in the acute setting as well as those in community settings after a period of time in hospital. This can be either in health beds whilst the person is benefitting from rehab or in the person’s home. We also work with those who need support to prevent a hospital admission including playing a role at the front door of the hospital by working with our health colleagues in the Emergency Department.

We are a friendly and supportive team working in a busy environment; although working under pressure we maintain high professional standards of practice and are committed to ongoing professional development. The successful candidate will be expected to work across all settings in line with the changing demands of the system to enable discharge from our service in a safe and timely manner.

About the role

About You

We are looking for a full-time qualified social worker who has strong assessment and communication skills, an excellent knowledge of relevant legislation, such as the Care Act and Mental Capacity Act, and someone who enjoys working in a fast-paced, multi-disciplinary setting.

Alongside our health colleagues the successful candidate will:

  • Work with individuals, families and carers in accordance with relevant statutory legislation and will embed consideration of strengths and relationship-based practice into their assessment processes.
  • Be confident in their knowledge and experience of safeguarding vulnerable adults and the assessment of risk.
  • Understand the importance of information and advice in supporting people to maintain their own independence and wellbeing.

Your starting salary will be pro rata if the above position is part-time or term-time only.

Work Permits: we may be able to obtain a Work Permit for this post but this is subject to meeting the requirements of the UK Visas and Immigration (UKVI) Points-based Immigration System.
